CMS-1500 Claim Form

The CMS-1500 claim form is a fact of the American medical way of life. It is a form that is universally used by providers to make claims with insurance companies and Medicare/Medicaid.  Synapse (R213B3) now has a tab dedicated to this form at Patients/Details/CMS-1500.  If no data is entered into the form there, Synapse will automatically populate it with as much information it can.  Thereafter the provider should fill in the rest which will be used as defaults for printing out this form.

Just enter all the other details you wish to appear as default and then click on [Save].  Thereafter these values will appear when this tab is viewed.

Each of the fields shown has a specific name, and so this allows us to access these values when creating a print template as in Settings/Forms/PDF Forms.
