To create and print a Superbill, you need to first need to make sure that you have the free postscript fonts installed.

The Synapse EMR superbill has the following components

  1. Patient details

  2. Procedures tracking

  3. Office injections administered

  4. Radiology ordered

  5. Laboratory orders

  6. Diagnosis/Assessment

  7. Level visit

  8. Follow up

To allow Synapse EMR to create the Superbill automatically for you, you need to use the various templates that include the CPT and ICD9 codes. These can be downloaded from the online library from the Settings/Templates tab, using the “Online Library” button.  They may  already be included in your distribution for you.

These templates can then be altered to suit your own practice. At the time of this writing, the ASSESSMENT template does not have the ICD9 codes associated with them. You can add them yourself by editing and then saving the template.

Let’s take a walk through on creating a Superbill. If you don’t have the templates referred to, you will need to download them as above.

  1. Select a patient

  2. Create a new consultation or encounter ( Patients/Consults – “Add New” -> “Followup

  3. Click on the blank consult to enter the consult editor

  4. Select the Template tab, and then the  "P" button -  "P" standing for  Plan.

  5. Choose the template named “INVESTIGATIONS: - blood CPT”

  6. Tick a couple of blood tests, and then “Save”

  7. Click on the “ASSESSMENT: with ICD9” template

  8. Tick on a diagnosis, or use the “Lookup” button to search for an ICD9 code

  9. Save it
  10. Click on the “Superbill” button

  11. Select the visit level

  12. Select the appropriate follow up period

  13. Then use the “Print Superbill” to preview the printout.

  14. If your preview is largely blank, try using the “100%” button. If it then appears, it means you have failed to install the PostScript font package correctly

  15. With the Superbill visible, you can either send it to the network PostScript printer, or create a PDF for printing,  or use  GSPrint to send it to the  default windows printer.

The superbill can also associate CPT codes with a diagnosis, and then export this to a billing package.  Currently Zybex is the only package that supports this functionality.

At the top right of the Superbill, there is a table of the diagnoses associated with this visit as generated by the Assessment template.  You can pick more diagnoses from the patient's existing diagnoses by double clicking on one of the items on the Existing Diagnoses table.  To remove items from the right hand table, you right click on them.

Let's first allocate the primary diagnosis for the visit level.  Ensure that your visit level has been selected, then click on the diagnosis from the top right table, and then click on the large green button in the middle of the screen.  If you make a mistake, you can right click on the item in the HCFA/CMS-1500 table to remove it.

Now switch to the "CPT Codes Used" tab.  Click on a CPT code to add it to the table below, and then click on each diagnosis associated with that CPT code.  You are allowed up to 4 diagnoses per CPT code.  Once you have used up all the CPT codes, you can then export the superbill for further processing.
